   用透明的角膜片置换混浊或有病变部分的角膜,以达到增视、治疗某些角膜病和改善外观的目上。是异体移植效果最好的一种手术。分两种:①全层(穿透性)角膜移植术  。以全层透明角膜代替全层混浊角膜。适应症包括中央性角膜白斑、角膜变性、圆锥角膜、顽固性角膜炎或溃疡及角膜瘘等,这种手术要求移植片内皮细胞有良好活性,故最好取自死后数小时内摘时的眼球,手术原则是根据病变范围选择适当口径的角膜环钻,分别作术眼及供眼角膜切除,作成移植床(术眼)及移植片(供眼),将移植片置于移置床上,缝线固定。术终可注气或林格液以恢复前房。手术成功的关键是不伤害术眼眼内组织及移植片内皮。并使移植片与移植床对位吻合良好。②板层角膜移植术。将浅层角膜病变组织切除,留下一定厚度的角膜作移植床,用一块同样大小和厚度的板层移植片放在受眼角膜床上。以间断缝线固定,植片和植床必须平整及互相吻合,才能得到良好的光学效果。适应症包括中浅层的角膜斑翳或营养不良性混浊、进行性角膜炎或溃疡、角膜瘘、角膜肿瘤等。因手术不穿通眼球,故较安全,并发症少,但光学效果不如穿透性角膜移植术。
