放线菌感染于腹腔脏器而发病。放线菌多由肠道而来，但也有时自胸腔过膈肌而致腹腔。急性初发时似阑尾炎，手术所见病变亦与化脓性阑尾炎相同，但手术后伤口甚难愈合，终成瘘管，经常流出脓液，内含硫磺样颗粒。慢性腹部放线菌病，常见症状为腹痛、腹泻或下坠感等。经数月后则在右骼窝部发生硬性肿物，稍痛或无痛感，表面皮肤渐成红色或灰红色。有时肿物刺激腰大肌或腰丛引起大腿部的反射痛。
