| 肺痈 以咳嗽、胸痛、发热、咳痰粘稠腥臭，甚则咳吐脓血为主证的肺部痈疡。多由于人体正气不足，或饮酒无度，恣食肥甘以致痰热内蕴而感受风热病毒，或风寒化热入里，内外合邪，热毒内蒸，热壅血瘀，肉腐血欺，则化脓成痈。肺痈的初期为外邪犯肺，邪未正表，病在肺卫，证见发热恶寒 ，咳痰粘而少，胸闷疼痛，口干鼻燥，苔黄脉浮滑而数。治宜疏散风热，清肺化痰，方用银翘散加味。成痈期为热壅血瘀，热毒内盛，正邪交争阶段，证见为热寒战，胸闷咳嗽，吐痰腥臭，苔黄腻，脉滑数。治宜清热解毒，化瘀。方用千金苇茎汤加味。溃脓期为热毒内蒸，痈脓内溃，证见咳吐脓血 ，气味腥臭，胸中烦满，身热面赤，烦躁，舌红苔腻，脉滑数。治宜清热，解毒，排脓，方用桔梗汤合千金苇茎汤。恢复期为邪去正虚，气阴两虚。证见咳痰减轻，胸痛缓解，身热渐退，兼神疲纳呆，舌红少苔，脉细无力。治宜益气养阴，润肺，方用清燥救肺汤加味。 由于肺与大肠相表里，大肠的通畅，有助于肺气宣降 。故在用药时应注意到这一点。儿童、老人及体弱者，预后较差。 |
