腐蚀剂损伤 caustics,injury due to 因强酸(硫酸、盐酸、硝酸)和强碱(氢氧化钠和氢氧化钾)等腐蚀剂对皮肤和粘膜的强烈腐蚀,致使蛋白质凝固,产生局部皮肤粘膜的灼伤和坏死。口服强酸或强碱,可引起口腔、食道及胃肠发生病理变化,如充血、水肿、坏死及溃疡,继而发生穿孔,最终形成瘢痕、狭窄及畸形,一般强碱灼伤较强酸灼伤严重。 口服强酸后应立即饮用或向胃内灌入氢氧化铝凝胶60毫升或极稀薄的肥皂水100~200毫升,内服强碱后应尽速服用食醋、3%~5%醋酸或稀盐酸,继而服用牛乳、豆浆或鸡蛋清以保护创面。禁忌催吐和胃管洗胃。禁食,静脉补充液体和营养。皮下注射吗啡或杜冷丁止痛。皮肤和眼部灼伤,应立即用大量清水或生理盐水冲洗5~10分钟。酸灼伤皮肤部位可用1%氨水或4%碳酸氢钠溶液洗涤或湿敷。碱灼伤可采用2%硼酸溶液洗涤或湿敷。然后分别按皮肤和眼烧伤进行常规处理。早期采用肾上腺皮质激素有助于防止瘢痕形成。 |
