| 腐蚀剂损伤 caustics，injury due to 因强酸（硫酸、盐酸、硝酸）和强碱（氢氧化钠和氢氧化钾）等腐蚀剂对皮肤和粘膜的强烈腐蚀，致使蛋白质凝固，产生局部皮肤粘膜的灼伤和坏死。口服强酸或强碱，可引起口腔、食道及胃肠发生病理变化，如充血、水肿、坏死及溃疡，继而发生穿孔，最终形成瘢痕、狭窄及畸形，一般强碱灼伤较强酸灼伤严重。 口服强酸后应立即饮用或向胃内灌入氢氧化铝凝胶60毫升或极稀薄的肥皂水100～200毫升，内服强碱后应尽速服用食醋、3％～5％醋酸或稀盐酸，继而服用牛乳、豆浆或鸡蛋清以保护创面。禁忌催吐和胃管洗胃。禁食，静脉补充液体和营养。皮下注射吗啡或杜冷丁止痛。皮肤和眼部灼伤，应立即用大量清水或生理盐水冲洗5～10分钟。酸灼伤皮肤部位可用1％氨水或4％碳酸氢钠溶液洗涤或湿敷。碱灼伤可采用2％硼酸溶液洗涤或湿敷。然后分别按皮肤和眼烧伤进行常规处理。早期采用肾上腺皮质激素有助于防止瘢痕形成。 |
