根据肾素-血管紧张素-醛固酮三者之间的密切关系提出的一个新概念。其作用首先为肾素把血浆中的血管紧张素原(在α2球蛋白中）水解生成血管紧张素Ⅰ(10肽）它能刺激肾上腺髓质使之释放肾上腺素。但对血管的收缩作用较弱。血液和组织中，特别是肺组织中有转换酶，这种酶可使血管紧张素Ⅰ降解，生成血管紧张素Ⅱ(8肽）。后者可进一步被氨基肽酶水解为血管紧张素Ⅲ(7肽）。血管紧张素Ⅱ是一种生物活性很高的升压物质，并可使肾上腺皮质增加醛固酮的合成和分泌醛固酮，又可促进肾小管对钠的重吸收，具有保钠和保水的作用，使细胞外液量增加，血管紧张素Ⅲ也有缩血管和促进醛固酮合成和分泌的作用，但血中的浓度低。所以血管紧张素Ⅱ的作用是主要的，因此三者之间密切联系，不仅有生理理论意义，而且也用于临床解释某些水钠潴留现象，例如肝硬化，出现水肿和脱水后，常伴有继发性醛固酮增多症，其机制则与本系统有关。
