pseudomonas aeruginosa pnenmonia
一种坏死性支气管肺炎，多发生于心肺等严重病症患儿、早产儿、粒性白细胞缺乏或免疫缺陷的患儿，以及长期用抗生素治疗的患儿。临床特点：①出现寒战、中等度发热，早晨比下午高，中毒症状、咳嗽、呼吸困难和发绀；②排出大量脓性绿色痰液，可有咯血；③脉搏与体温比较相对缓慢；④肺部体征无明显的大片实化，有弥漫细湿■音及喘鸣音；⑤轻度白细胞增高，但1/3病人白细胞可减少，并可见贫血及黄疸；⑥X线胸片可见结节状浸润阴影及许多细小脓肿，后可融合成大脓肿；一侧或双侧出现少量血性胸腔积液或脓胸。⑦痰内可见大量革兰染色阴性杆菌。病情发展迅速，病死率极高，达40%。因细菌多而耐药，疗效差，近年多选用羧苄青霉素、磺苄青霉素、庆大及丁胺卡那霉素。近年来采用三代头孢菌素如头孢胺噻肟(复达新）有效。
