腹膜外尿道悬吊术


腹膜外尿道悬吊术是矫治张力性尿失禁的方法之一。张力性尿失禁是指在一般情况下能控制排尿,但当腹压突然升高时,尿液便不自主排出。发生张力性尿失禁的常见原因是尿道后壁与膀胱三角底部形成的尿道后角消失。腹膜外尿道悬吊术多用于张力性尿失禁经阴道手术失败或复发的病例。该术式特点是经腹膜外,耻骨联合后,游离膀胱颈和尿道,再分别将尿道旁组织及膀胱用丝线固定于耻骨联合后骨膜及腹直肌后方,恢复尿道后角。经腹膜外尿道悬吊术主要的并发症有,耻骨联合后血肿、脓肿及骨膜炎等。因此,手术操作时应实施严密的止血及严格的无菌技术。
