消痞化积

消法之一，用行气化瘀，消滞软坚的药物，以消除痞积的治法。痞，一般是指痞证，是邪气结于心下，心下痞满而不痛的自觉症状，但无有形之积块。积，是指积证,为寒热饮食与气血相搏，聚而不散，积而成块,为有形之积块。
消痞 主治痞证，表现为心下痞满而不痛，不欲饮食等。多由脾胃虚弱,中焦不运，升降失常,寒热互结于心下所致。治疗宜和胃降逆、行气消痞，常用半夏、干姜、枳实、黄连、黄芩、党参、大枣等药。代表方剂如半夏泻心汤，枳实消痞丸。
化积 主治积证，主要表现为腹内之结块，如肝之积肥气、脾之积痞气、疟母以及肿瘤等。化积法常根据病情把行气活血、化痰利水、软坚散结、清热解毒等不同作用的药物配合使用。常用的药物有三棱、莪术、五灵脂、 桃仁、红花、鳖甲、麝香、香附、 枳壳、青皮、天花粉等。代表方剂如鳖甲煎丸、膈下逐瘀汤、犀黄丸等。
积块日久，人体正气受损伤，致正气虚弱，须配合补法治疗。临床辨别气、血、阴、阳以及脏腑衰虚的具体情况分别予补益的方法。如气血不足，可配合八珍汤，脾胃虚弱可配合六君子汤。
临床注意事项 痞积日久，损伤正气，故在治疗上始终注意保护正气，攻伐之药用之不宜过度邪衰应扶正达邪,以免伤正。正如《素问·六元正纪大论》说:“大积大聚，其可犯也，衰其大半而止。”
