| 胃切除术 gastrectomy 治疗胃肿瘤、胃十二指肠溃疡及其并发症的手术。有关胃的切除范围及其手术方式需根据病变部位及性质来决定，其中胃大部切除术是最为常见的手术，对胃及十二指肠溃疡病的治疗，经近60余年的实践考验，有效治愈率达到80％～90％以上。手术的理论根据是降低胃酸分泌，切除病灶。传统的方法需切除胃远端的2／3～3／4。代表性的吻合方法有比尔罗特氏Ⅰ式和Ⅱ式二大类，前者是将剩余胃直接与十二指肠吻合，较符合生理，但往往因顾虑吻合口张力过大而切除范围不够造成术后溃疡病复发是其缺点；后者是残胃与空肠吻合，复发率较低，但术后胃肠功能紊乱的因素增加。胃切除术后的主要并发症有吻合口狭窄、出血、穿孔、溃疡复发、倾倒综合征等。远期并发症可有营养不良、贫血、胆囊炎、胆石症及残胃癌等。 |
