由于碱基替换、导致密码子意义改变，在合成蛋白质时一个氨基酸被另一个氨基酸代替，使合成的蛋白质失去活性或活性降低。如人类的镰形细胞贫血病，是由于患者的血红蛋白(s）β链上第6位置的谷氨酸为缬氨酸所代替。分子机制谷氨酸在DNA上的三联密码是CTT，由于突变，成了CAT，结果缬氨酸代替了谷氨酸，导致血红蛋白输氧能力的降低；红细胞呈镰形。
