    发生于18岁以前的儿童和少年的各种行为和情绪异常。儿童行为障碍包括两个方面,一是儿童常见的生理心理行为偏异,如遗尿、厌食、偏食、夜惊、睡行、口吃等;一些是习惯性动作,如吮手指、咬指甲、习惯性抽动(如习惯性眨眼、咂嘴、扭头、耸肩等)。儿童的不良社会行为,属于儿童品行障碍,不属于行为障碍。儿童情绪障碍指的是儿童情绪反常,如过分害羞、恐惧、焦虑、暴怒发作等。行为障碍和情绪障碍在儿童中很常见,随着年龄的增长通常会自行改善,应正确认识和处理这些现象,促进儿童身心健康成长。
   儿童行为障碍发生的原因包括:①多数与暂时性生理或心理发育延迟有关,例如随着年龄的增长和发育成熟,大多数遗尿会自然消失;②咬指甲、习惯性抽动等行为障碍属于习惯性质,开始是偶然发生的,但由于不断得到强化,便逐渐固定下来,形成习惯;③厌食、偏食、吮手指等行为障碍的发生,与环境影响不良、教养不当有关;④少数行为障碍是器质性疾病的结果,例如不到10%的儿童遗尿常见的器质性病因有隐性脊柱裂、膀胱及尿道发育异常或慢性炎症等。儿童行为障碍的治疗强调医生、家长和教师的共同努力。改善环境和教育方法常可收到积极的效果。行为治疗对遗尿、口吃、习惯性动作有较好疗效。药物治疗有辅助价值,如丙咪嗪治疗遗尿有时有效。
   儿童情绪障碍常以单一的症状为主,相对轻微,随着年龄的增长常可自行改善。表现:①过分害羞。怕见生人,怕到新环境,爱脸红,易哭泣,胆小,拘谨,行为退缩;②情绪不稳定。容易激惹,不顺心时发脾气,哭叫吵闹,毁坏东西,撒泼打滚,严重时还表现为迸气发作,可伴发紫绀和痉挛;③过分恐惧。很多幼儿怕黑暗、怕生人、怕动物、怕雷电、怕鬼怪等,但随着年龄的增长会自行改善;如果恐惧程度较重,持续时间较长,给儿童本人和家长带来困扰,便属于情绪障碍;④有的儿童容易烦恼,紧张不安;有的可出现分离性焦虑,即与亲人分离而引起的焦虑反应。患儿依恋父母,寸步不离,拒绝上幼儿园或学校,并可出现头痛、恶心、呕吐等躯体症状。儿童情绪障碍的发生通常与精神刺激有关,如父母离异、亲人死亡、惊吓、恶劣环境等,慢性躯体疾病和某些气质因素可增加情绪障碍的易感性。治疗包括调整环境,正确教育,行为治疗和心理治疗,大多数患儿不需药物治疗。
