肾小囊壁层上皮细胞大量增生,在肾小囊内形成半月体或环状体。半月体一旦形成很难吸收,在短期内可出现严重纤维化。半月体的形成使肾血管球受到不同程度的挤压。广泛严重的半月体形成(占全部肾小球的50%以上)造成肾小球的堵塞,引起尿闭、急性肾功能衰竭,临床称之为“急进性肾小球肾炎”。电子显微镜下在半月体内见有大量纤维素沉着和上皮细胞增殖,可见不同程度血管袢细胞增殖、栓塞和坏死。内皮细胞及上皮细胞下或系膜内可见电子致密物的沉积。
