high anomalies of anus and rectum
为先天性肛管直肠畸形的一种类型。其直肠盲端终止于肛提肌之上。分两个亚型:①肛管直肠发育不全。再分无瘘管、有瘘管两大类。在男性有直肠膀胱瘘、直肠尿道瘘,在女性有直肠膀胱瘘、直肠泄殖腔瘘、直肠阴道瘘。②直肠闭锁。患儿无肛门,可有胎粪从阴道或泄殖腔排出,或尿中有粪质。新生儿期有低位完全性或不完全性肠梗阻症状为与体征。倒立侧位X线平片可见直肠末端气影在耻尾线之上。稍大小儿膀胱尿道造影可显示直肠泌尿系瘘。治疗为新生儿期行横结肠造口术,解除肠梗阻。6个月后行腹骶会阴直肠成形术,同时结扎瘘管。后再行横结肠关口术。亦可于新生儿期行腹骶会阴直肠成形术。
