多由丝虫病淋巴管病继发溶血性链球菌感染引起阴囊淋巴管阻塞。淋巴液从扩张破裂的淋巴管溢出，刺激阴茎、阴囊纤维组织大量增生所致。阴茎、阴囊皮肤增厚，皮脂腺破坏，故局部皮肤干燥似象皮而无弹性，严重者可重达数十千克而影响活动，阴茎会内缩而影响性生活。除治疗原发病外，严重者可行手术切除象皮肿及阴囊成形术。
