分娩心理调节(mental adjustment duringparturition)对分娩时的恐惧心理进行调节可以稳定孕妇情绪,有助于分娩的顺利进行。在一些国家中曾流行过两种心理调节的方法:(l)里德法由英国医师格兰特利·迪克一里德在1944年出版的《无恐惧分娩》一书中曾作了介绍。里德认为,造成正常分娩极度疼痛的主要原因是恐惧。对预期疼痛的恐惧引起子宫肌肉收缩,这对宫颈口的扩张产生了抵抗,由此而来的紧张被产妇解释为疼痛,结果又提高了她的恐惧和紧张,进一步加剧了疼痛,以致恶性循环,严重者甚至会造成难产。里德提出的力一法是:让孕妇学习和了解怀孕、临盆、分娩的现象,消除由于对分娩过程的无知和焦虑引起的恐惧一紧张~疼痛~恐惧的恶性循环。(2)拉马兹法。最初出现于俄国,被法国医师费尔南德·拉马兹引进法国,后又在美国普及。该方法基于巴甫洛夫条件反射的原理,把疼痛的感觉与分娩分离开,使分娩中的子宫活动及其他生理反应被条件化为分娩的感觉,而不是被体验为疼痛。分娩时孕妇就能把身体感觉与平时训练得到的无痛感觉结合起来,进而调整呼吸和配合医师助产。 (陈若莉撰荡会舟高玉样市)
