经颅骨锥孔或钻孔用脑室穿刺针直接穿刺至侧脑室。用于脑室造影诊断性检查和脑室引流。梗阻性脑积水引起颅内压增高、脑疝危象时，迅速行侧脑室穿刺术，放出部分脑脊液，可使颅内压立即降低，是一种有效的急救措施。
