脊髓前外侧束切断术是指切断脊髓的前外侧束，以阻断痛觉传导到脊髓丘脑侧束，从而达到止痛的目的。其切割平面的选择如下：①疼痛上达第5颈皮节者切割对侧颈2平面；②躯干上部及下肢痛切割对侧胸口平面；③疼痛部位位于中线或双侧者，可行双侧脊髓前外侧束切断。但它常引起下列并发症：①术侧轻偏瘫；②括约肌与性功能障碍；③双侧同时切断者还可发生自主性功能障碍。
