纵隔炎
纵隔的急、慢性炎症，可由多种病因所引起，大多属继发性。临床分急、慢性两种。急性纵隔炎较少见。主要由食管穿孔所致。由于炎症广泛，纵隔具有较强的吸收能力，故常引起严重的全身中毒现象，如高热、嗜睡、末梢循环衰竭和血液白细胞增多等，常伴有胸骨后疼痛，向颈部放射，还可出现吞咽困难、声音嘶哑等症状。慢性纵隔炎包括肉芽肿性纵隔炎和硬化性纵隔炎。病因多不明确，肉芽肿性纵隔炎约1／3属结核、真菌或结节病所致，以纵隔慢性肉芽肿为特征。硬化性纵隔炎又称特发性纵隔纤维化，常找不到明确病因。病理上以纵隔纤维性肿块和纵隔结构粘连为特征。本病临床表现常较轻，多数病例无症状。结核性者常有低热、盗汗、乏力等中毒症状。部分病例可因肉芽肿压迫或纤维性粘连而引起上腔静脉综合征和吞咽困难等相应的表现。
