测定直肠、肛门括约肌的静止压力分布、肛管直肠静止压力差、直肠肛门内括约肌的反射性压力变化,对鉴别便秘的原因,诊断先天性巨结肠,了解治疗肛门失禁手术后肛直肠功能的恢复等甚有价值。常用方法有气囊测压法、液体灌注测压法。正常儿童,在直肠壶腹膨胀时会出现肛门内括约肌的松弛。而在先天性巨结肠患儿,因直肠及内括约肌部的肌间神经丛和黏膜下神经丛内缺乏神经节细胞,故正常的直肠肛门内括约肌松弛反射消失。在测压图上可见,在直肠气囊中注入气体膨胀刺激直肠后,肛门内括约肌部位的压力没有下降,甚至反而上升。这一现象为直肠肛门内括约肌松弛反射消失。
