| 肺痿 因肺热灼伤津液或肺虚气不化津而导致肺叶枯萎，临床以咳吐浊唾涎沫为特征的病证。肺阴不足是其主要病机。多由外感热邪，耗伤津液，或痨嗽日久，肺阴消耗，或大病久病，耗伤阳气，气不化津，津液不布等引起。肺痿也可由其他肺部疾患变化而成。如肺痈，肺痨，咳喘等。临床当明辨虚热和虚寒。①虚热肺痿。为肺有燥热，津液灼伤，证见咳吐浊唾涎沫，咽干声哑，气急喘促，形体消瘦，皮毛干枯 ，舌红干，脉虚数。治宜滋阴清热润肺。方用麦门冬汤。②虚寒肺痿。为肺气虚寒，气不化津，证见咳吐涎沫，色白清稀而量多，神疲乏力，心悸气短，形寒肢冷，舌淡脉弱。治宜温肺益气，方用炙甘草汤加减。虚寒肺痿和虚热肺痿是相互转化的，虚热肺痿，日久不愈，阴损及阳，可转为虚寒肺痿，反之亦然。总之，肺痿是肺之虚证，也是肺病中较难治的一种病。治法以滋阴润肺、温肺益气为主，慎用峻猛之剂。 |
