脊髓蛛网膜炎
spinal arachnoiditis
又称脊髓蛛网膜粘连。一般认为是继发于某种致病因素作用下产生的组织反应。如病毒感染、脊髓损伤、多次药物鞘内注射或髓外肿瘤及脊髓关节炎刺激等。脊髓蛛网膜炎多属慢性疾病，病程中症状时有缓解，但迅速起病或亚急性者均有，根据病变发展情况分为局限型和弥漫型两类。局限型往往有急性感染史、高热，随即出现根痛及较明显而固定的感觉障碍，有的有运动障碍；弥漫型一般病程缓慢，病程中逐渐出现感觉异常、过敏、感觉麻木、进行性肌无力或瘫痪。脑脊液改变，细胞正常或轻度增加，蛋白含量可增加，压颈试验多呈椎管不完全梗阻征象。脊髓造影可见造影剂呈散在点状或烛泪状表现，治疗可用激素、抗生素、B族维生素及血管扩张剂等。瘫痪时要加强护理，预防尿路感染及褥疮。
