膀胱输尿管反流

膀胱壁段输尿管只有纵形纤维。这些肌纤维延续到输尿管口进入三角区,并于此处与对侧者相会合,形成输尿管三角区复合体,作为一个功能单位。各种原因如先天闭锁不全、尿路感染、下尿路梗阻、其他尿路畸形等致该单位功能障碍,即可出现膀胱输尿管反流。反流可致肾、输尿管积水、肾萎缩及发育停滞。按膀胱内压情况可分为低压力反流与高压力反流。也可按反流程度分为:①尿液反流到下段输尿管;②输尿管及肾盂肾盏充盈但无扩张;③输尿管、肾盂、肾盏充盈,肾盏呈杵状,肾盂轻中度扩张,但无输尿管迂曲;④输尿管严重迂曲及扩张。对原发输尿管反流首先考虑保守治疗,只有在足够的药物治疗后感染仍复发以及上尿路有病变时,才考虑防止反流的手术。
