病因未明,可能是靶细胞甲状腺激素受体或受体后的缺陷所致。分甲减型:病情最重,血甲状腺激素浓度(T3、T4、FT4)增高而伴甲减表现,甲状腺肿大,可伴先天性聋哑,骨化中心出现及骨骺愈合延迟;代偿型:病情较轻,血中甲状腺浓度增高(T3、T4、FT3和FT4)。甲状腺功能正常,智力正常,无聋哑和骨骺愈合延迟。治疗不宜使用抗甲状腺药物,因可加重生长停滞及甲状腺肿的症状和体征,同时慎用外源性T3和T4,避免继发性垂体增生或多发性垂体肿瘤的发生。
