腹股沟直疝
direct inguinal hernia
指经直疝三角突出的腹股沟疝。发病原因：直疝三角是一特别薄弱区；年老体弱，腹壁肌肉、腱膜退化，腹壁强度降低，在腹内压增高的作用下可发生直疝。临床上多见于老年体弱者，属后天性疝。站立时疝块位于耻骨结节外上方，半球状；平卧后疝块自行消失。疝环宽大，极少嵌顿；扪诊直疝三角腹壁有缺损。根据下述任何一点可与直疝鉴别：①疝块从不下降阴囊；②回纳疝块后按压内环，疝块仍然突出；③术中探查疝环位于腹壁下动脉内侧。原则上应行直疝修补术。
