| 血吸虫尾蚴性皮炎 schistosome cercarial dermatitis 禽、畜类血吸虫的尾蚴侵入人体皮肤引起的一种变态反应性炎症。因常在水稻种植时发生，所以又称稻田皮炎。在许多国家，常因在淡水湖或半咸水海游泳后发生，故称游泳痒，日本人称湖岸病。在中国，从东北至广东，从东部沿海至西南，水稻种植区均见该病。能引起本病的血吸虫种类颇多，但在中国主要是寄生于鸭类的包氏毛毕吸虫和寄生于牛、羊的土耳其斯坦东毕吸虫的尾蚴。因人不是这些血吸虫的适宜寄主，尾蚴钻入皮肤后可于局部组织内被杀灭，即使进入血流到肺部，也不能存活。尾蚴侵入后分泌的蛋白酶及尾蚴死亡后释出的蛋白质和多糖均具抗原性，可产生变态反应而引起皮炎。 该病多发生于与水面接触的皮肤部位，与尾蚴常在水面活动的习性有关。在稻田劳动时罹致的皮炎，主要发生于两小腿、两前臂及手背、足背等部位。皮肤与含有尾蚴的疫水接触后，数分钟或数小时内即发生剧痒，继而出现红斑和较硬韧的丘疹，周围有明显的红晕；严重者，丘疹扩大融合成风团块，甚至形成水疱。然后，症状逐渐消退，脱痂。因患部奇痒被搔破可继发感染或糜烂。若反复多次接触疫水受尾蚴感染后，皮炎症状也愈加严重。一般而言，禽类血吸虫尾蚴引起的皮炎症状较畜类血吸虫尾蚴引起的更为明显而剧烈。治疗原则主要是止痒、抗过敏和防止继发细菌感染。止痒药可用复方炉甘石洗剂、薄荷酚洗剂等；重者加用抗组胺药或其他镇静药。 |
