癣 tinea 由皮肤癣菌,主要是小孢子菌、毛癣菌和表皮癣菌引起的传染性皮肤病。又称皮肤癣菌病。侵犯表皮角层、指(趾)甲、毛发,有时还侵犯角层以下深部组织。癣的发病率很高,有传染性,通过直接和间接接触传染。可继发细菌感染、秃发、手足皲裂等。皮肤癣菌病一向是按解剖部位命名,如头癣、体癣、股癣、手癣、足癣、甲癣等。如按病因命名可分类为,红色毛癣菌病,可侵角层,引起体、股癣和手、足癣,侵甲引起甲癣,侵毛发引起头癣、须癣,还可侵深部组织引起脓癣、脓肿。临床表现在皮肤有丘疹、水疱、鳞屑组成的环形或多环形红斑损害。甲板增厚、不平、萎缩或缺损。毛发折断、参差不齐,有时可有菌鞘。拨发或刮屑镜检,检到菌丝孢子即为阳性,做培养可以鉴定菌种。治疗可外用克霉唑、咪康唑、益康唑、联苯苄唑等。目前新药三并萘芬治疗甲癣较好。 |
