晕厥 syncope 因一时性广泛的脑供血不足而突然发生的短暂意识丧失状态。患者肌张力消失,倒地或不能保持正常姿势,可于短时间恢复。正常人脑血流量高于身体其他组织,成人血流量每分钟为45~50毫升(mL)/100克(g)脑组织。维持意识所需脑血流量的临界值为30mL/100g/分,当脑血流量骤减至临界点以下时,即可引起晕厥。导致脑血流量骤减的原因是:①血压急剧下降。为周围血管张力的反射性丧失,周围小血管迅速广泛地扩张所致,为反射性晕厥,血管迷走性晕厥、颈动脉窦性晕厥、排尿性晕厥、直立性低血压性晕厥、咳嗽性晕厥等属之。②心排血量降低。常由急性心功能不全或血容量急剧减少所致,为心源性晕厥,见于心律失常(包括心搏过速、心搏徐缓、心室停搏)、心肌病变、急性心脏排出受阻(左心房粘液瘤、心瓣膜病变、先天性心脏病)、肺血流受阻(肺动脉栓塞)等。③供应脑部血流的动脉急性缺血。多由脑血管有闭塞性疾病时,脑血管阻力增加所致,为脑源性晕厥,见于主动脉弓综合症。 |
