视网膜脱离 retinal detachment 视网膜神经上皮层与色素上皮层的分离。可分为:①原发性视网膜脱离。多见于近视眼患者及老年人,视网膜常有囊样变性、玻璃体液化、玻璃体脱离或玻璃体内有索条与变性的视网膜相粘连,引起视网膜破裂,这时液化的玻璃体经裂孔流入视网膜下腔而引起视网膜脱离,病人发病前感到眼前有小黑点、闪光感,以后视力突然下降,视野中有缺损。眼底检查可见脱离的视网膜及破孔,眼压低于正常。采用手术治疗。②继发性视网膜脱离。继发于全身病或眼部疾患,分渗出性(多见于高血压、肾病或妊娠高血压综合征、原田氏病等)、牵引性(由各种原因引起的玻璃体机化索条牵拉视网膜)及实体性(多由肿瘤引起)。治疗以处理原发病为主,控制病因,促使渗出液吸收及切除玻璃体机化索条等。
