皮肤与黏膜因表面小血管内血液还原血红蛋白绝对值增高而表现为青紫色。中央性发绀是由于动脉血氧饱和度降低(＜85%）所致，见于右向左分流的先天性心脏病、肺内血循环不足以及肺换气障碍的疾病中，发绀较严重。周围性发绀系由于毛细血管内血液流动缓慢、组织摄氧量增加所致，发绀较轻，其原因为动脉血氧未饱和度增加，周围毛细血管中过量的血红蛋白被还原和异常血红蛋白衍生物产生。
