即是膈肌腰部与肋部结合不全，使大量腹腔脏器如肠、胃、脾、肾进入胸腔，常无疝囊。由于右侧有肝脏的关系，故左侧发生较多。临床表现：由于腹内脏器大部进入胸腔，压迫心、肺，特别是胎儿期已造成心脏畸形或肺发育不全者，引起严重呼吸，循环障碍，故多于新生儿期出现症状。表现为气急、心悸、呼吸困难及发绀，喂乳及哭闹时加重，卧于患侧或半坐位时则稍可减轻。由于腹内器官进入胸腔，腹凹陷呈舟状，宜手术治疗。
