又称“肾小球滤过屏障”。流经肾的血液经此膜滤出,形成原尿。滤过膜由三层结构组成。内层是毛细血管内皮细胞层,有大小不等的微细小孔,称内皮窗,大于内皮窗的血浆蛋白将受阻而不能通过此层;中层为基膜,基膜较厚,是由水合凝胶构成的,其中无孔,但有微纤维网。基膜分为电子密度不同的三层,即外疏松层、致密层及内疏松层,有些较大的蛋白质分子可透过内皮窗,但却受阻于基膜;滤过膜的外层,是肾小囊的脏层上皮细胞层,具有足突,足突之间有裂隙称为裂孔。裂孔上有一层滤过裂孔膜,是物质滤出的最后一道屏障。某些透过内皮窗及基膜的蛋白质还可受阻于足突裂孔。肾小球滤过膜除具有上述体积屏障作用外,由于肾小球基膜表面含有硫酸肝素等蛋白多糖,脏层上皮细胞足突表面富有唾液酸等物质而带有负电荷。故还能起到电荷屏障作用,使血浆中带负电荷的蛋白质不易通过。
