减张缝合
减张缝合即张力缝合。适应证：①年老体弱，消耗性疾病(如癌症)，因组织愈合能力较差，关腹时可加做减张缝合；②术后可能腹压过高、剧烈咳嗽、腹水、妊娠、肠麻痹等患者；③腹壁切口裂开再次缝合。常用腹膜外减张缝合，即先关闭腹膜，用大弯三角缝针，坚实的10号尼龙线，自皮肤贯穿腹壁筋膜下，每隔3～4cm一针，针眼距切缘2～3cm，暂不打结，将筋膜、皮下脂肪及皮肤缝合后，张力线套上长约2cm的橡皮管，或夹上小纱球，依次收紧打结。张口线一般保留10～14天。
