烧伤 burns 由热力、化学物品、电及放射性物质引起的组织损伤。热烧伤最常见,主要由各种火焰、热液产生的高温对组织造成的损伤;其次是化学烧伤,为各种化学腐蚀剂(如强酸、强碱)使组织脱水、蛋白凝固所致;电烧伤居第三位,电流流经组织后产热并使细胞膜结构改变所致。烧伤的面积和深度决定病情的轻重,通常所说的轻度烧伤,是指面积不足10%的烧伤。大面积烧伤多指50%以上的烧伤。同样面积的烧伤,创面越深,伤情越重。临床一般将烧伤分为3度。Ⅰ度烧伤,只伤及皮肤表皮,伤区为红斑,轻度肿胀,灼痛明显,勿需治疗,1周自愈。浅Ⅱ度烧伤,伤及整个表皮及真皮浅层,伤区肿胀明显,有大量水泡,水泡破溃去除后露出潮红创基,疼痛明显,如处理得当,1~2周愈合,不留疤痕。深Ⅱ度烧伤,伤及真皮深层,水泡少,创面红白相间,疼痛轻,3~4周可愈合,多有疤痕。Ⅲ度烧伤,皮肤全层损伤,重者深达皮下、肌肉、骨骼,甚至内脏,创面呈瓷白色、焦黄或炭黑色,质硬如皮革,无痛觉,非手术不能愈合。治疗:轻中度烧伤以清创、包扎、暴露疗法为主,浅Ⅱ度、深Ⅱ度创面多能愈合;深度创面除上述处理外,尚需尽快行切痂、削痂植皮、皮瓣覆盖等手术。如创面陈旧,感染重,则应保痂、换药,待后行肉芽创面植皮。对大面积烧伤,应积极抗休克,抗感染,休克期度过后即予手术;营养支持及内脏并发症的防治也很重要。深度烧伤愈合遗留的疤痕,常造成毁容、畸形及功能障碍,多数需作整形手术方能改善。
