腹壁切口裂开
腹壁切口裂开往往是由于病人年老、体弱、营养不良,或病人突然用力如:恶心、呕吐、咳嗽、喷嚏、使劲用力排便等突然发生伤口裂开。表现为伤口有较多的淡红色液体流出,多数病人有崩裂感,如腹膜同时裂开可有大网膜及肠管脱出伤口,也有浅层裂开者如皮肤皮下组织裂开,多由于小血肿,脂肪液化等因素造成。预防:根据全身情况,有发生伤口裂开可能者,应在术时加用张力线缝合2~3针,伤口较大者应在术后包扎腹部与腹带,咳嗽时应由他人用手揿住切口两侧以减轻切口的张力。一旦发生伤口裂开应及时处理,有疑筋膜裂开,应及时在手术室清创缝合,并加缝张力线(详见减张缝合)。
