方法93例肾动脉狭窄支架置入术患者,术前术后以钙拮抗剂为首选的降压药物,配合使用血管紧张素转换酶抑制剂(ACEI)及肾上腺素能受体结合剂(ARB)类控制血压。

calcium antagonists
    主要通过阻断心肌和血管平滑肌细胞膜上的钙离子通道,抑制细胞外钙离子内流,使细胞内钙离子水平降低而引起心血管等组织器官功能改变的药物。临床常用的有硝苯吡啶、异搏定、硫氮唑酮等。对心脏的作用,主要是抑制心肌去极化过程中第二时相钙离子内流,降低细胞内钙,减弱心肌收缩力,降低心肌氧耗量,同时抑制窦房结和房室结的钙内流,使窦房结自律性下降,房室传导减慢,心室率降低,如异搏定,硫氮唑酮。在血管主要扩张动脉平滑肌,降低外周阻力,而对静脉平滑肌作用甚小,如硝苯吡啶。此类药物临床主要应用于高血压病、冠心病和心律失常。
