生理名。指妇女卵细胞的透 明带上存在特异的抗原,刺激机体产生抗透明带抗体,这种自身抗体与透明带起反应后,可 防止精子附着与穿透卵子,而阻止受精或防止囊胚从透明带逸出而阻断着床的特异性免疫反 应。从70年代开始,开展了卵透明带免疫引起不孕的研究,发现某些不孕妇女血清中确实存在有特异的透明带自身抗体。自从放射免疫测定卵透明带抗体方法建立后,可以更准确地了解血清中透明带抗体的含量。在特发性不孕的妇女中,有15%~30%的血清标本可测到透明带抗体。透明带自身抗体的产生机制还不清楚。一般认为,免疫细胞的自身识别是借助于免疫耐受的建立,在个体发生中的免疫耐受是在胚胎时期建立的,而透明带在胚胎时期尚未形成,因为始基卵泡并无透明带,它是在卵泡发育过程中出现的。所以从个体发生来看,机体对卵透明带的耐受尚未建立,这就给此物质的自身免疫打下基础。当每次月经来潮时,来源于闭锁卵泡的透明带物质,即使很微量,也可成为抗原。但这是女性的共同现象,不能说明只有不孕妇女才能产生抗体。所以不孕患者还可能是被一种不明来源的抗原所致敏,这种抗原与透明带有交叉反应,或由于不孕者有免疫调节细胞的失调参与其发病机制,此发病机制尚须进一步研究。检测血清卵透明带抗体,可用猪卵以荧光免疫法在荧光显微镜下观察,但血清必 须先用猪红细胞吸收、去除非特异性抗体,以避免假阳性。还可直接应用纯化的猪透明带抗 原以放射免疫法或酶联法进行检测其相应抗体。
