是牵引重物通过穿过骨的钢针(骨圆针或克氏针)或牵引器(颅骨牵引器)作直接牵引。有牵引力大、易护理的优点。常用的穿针部位有股骨远端、胫骨结节(股骨骨折、髋及骨盆骨折、脱位)、跟骨(胫腓骨骨折)、鹰嘴(肱骨髁上骨折)、颅骨(颈椎骨折、脱位)。牵引重量达5~15kg。
