风湿性二尖瓣狭窄的一种类型。除二尖瓣孔狭窄外，大、小瓣本身都有较严重病变，同时腱索也发生粘连、缩短，使瓣膜边缘约有1cm宽的组织受到牵拉，形成漏斗状。大瓣的大部分仍可活动，但受到一定限制。在听诊时，心尖区第一心音亢进，脆性多不明显，开放拍击声不清晰，甚至听不到；有心房颤动者较多。超声心动图示大、小瓣曲线稍增厚，可有轻度粗细不匀；大、小瓣活动幅度减少。宜选用二尖瓣直观成形术。
