性质:又称脑核性黄疸。未结合的胆红素具有亲脂性,对膜脂质有高度亲和力,同膜脂质结合后损害膜功能,特别是神经系统的功能。当血浆中未结合胆红素浓度超过20~25mg/dl时,它能透过血脑屏障,转移到膜脂质上,导致胆红素血症的毒性脑病,即胆红素性脑病,病人反应迟钝。由于胆红素代谢系统具有可诱导性,故给予新生儿黄疽病人苯巴比妥可奏效。此外,用可见光照射(光治疗)可通过尚不清楚的机制促使肝将某些未结合胆红素转变为可随胆汁排出的异构体,从而减轻毒性反应。
