血浆扩容剂


是一种用以维持和增加血浆容量的制品。由于急性大量失血,大面积烧伤和烫伤,剧烈呕吐和腹泻等原因,导致循环血量降低,引起低血容量休克时,为了迅速补足血容量,以改善微循环,有时可用人工合成的血浆代用品,即血浆扩容剂。对于这些制品的要求为:①有一定胶体渗透压,可在血管内保持血容量;②排泄较慢,但亦不应在体内持久蓄积;③无抗原性,不引起严重不良反应;④无细菌和致热源污染等。目前常用的有右旋糖酐、羟乙基淀粉、人血白蛋白、氧化聚明胶和聚维酮等。这类制剂多需用生理盐水或5%葡萄糖液溶解,葡萄糖溶液缺乏离子,易使水分子转入细胞内,导致尿量减少,对休克不利,适于不应摄入过多氯化钠患者。
