光镜下可见肾小球系膜细胞增生、系膜基质增加和扩展以及毛细血管壁增厚。电镜观察可分为3种亚型。Ⅰ型：增生的系膜细胞和基质伸入到基膜与内皮细胞之间，使毛细血管壁似乎分裂成双层而增厚，毛细血管腔被压而变窄；Ⅱ型：肾小球膜内出现电子致密沉积物，又称致密沉积物病；Ⅲ型：沉积物不规则地出现在毛细血管壁内。临床上本型多见于肾炎性肾病及慢性肾炎，血清C3持续明显降低，又称低补体血症性肾炎。
