动力学定向短程心理治疗(dynamicallyoriented brief psyehotherapy)在人为规定的约40次治疗限度内进行的精神分析心理治疗,其目的在于使病人对无意识冲突获得领悟。精神分析治疗一般需要在几年的时间内每周数次同病人见面,这种长期治疗花费大、耗时多,但效果不一定好。为此,于20世纪50年代和60年代人们就这个问题系统地调查了三个精神分析治疗中心:伦敦的塔维斯托克诊所、波士顿的马萨诸塞总医院和蒙特利尔总医院。结果发现,虽然这三家医院代表了不同的精神分析学派,在方法上有差别,但都认为可以根据己知的原则选择病人进行短程治疗,这些病人不一定是新发作的轻症病人,治疗方法亦非浮浅,经治疗后病人可获持久和巩固的疗效。动力学的短程治疗中选择病人的原则有:(l)必须从一开始就排除明显不合适的病人,如有强烈自杀倾向者、有严重抑郁或“精神崩溃”危险者、冲动控制力极差者、慢性酒精中毒或药瘾病人以及极端依赖者。(2)病人必须对焦虑有基本的面对能力,治疗结束后能独立生活。(3)病人必须把自己的问题看作是心理方面的,能同治疗者相互作用;不仅为解除症状而参加治疗,而且也愿意通过对自身的领悟而获得情绪上的成长。(4)病人的问题必须能形成一个可行的治疗计划或重点。在治疗方法方面,短程心理治疗要求:(l)治疗者必须提出一个有限的治疗目标并自始至终追求它。(2)主要治疗工具是阐释,即向病人解释他尚未意识到的情感。(3)主要的治疗技术之一是,向病人说明他对治疗者的情感是移情的结果。目前在美国等西方国家,动力学定向的短程心理治疗己被应用于一些精神病院和综合性医院的精神科、精神病急救机构、学校卫生机构、心理治疗诊所和私人诊所。 (梁宝勇撰徐俊见审)
