光镜下肾小球病变呈局灶性节段性肾小球血管袢硬化灶或玻璃样病变区，细胞可不发生明显增生，晚期可发展为球性硬化；电镜下可见弥漫的上皮细胞足突融合，并伴有系膜基质节段性增多；免疫荧光表现为局灶性巨块状或粗颗粒状IgM和C3沉积，有时阴性。临床表现为激素不敏感的肾病综合征。
