| 脱髓鞘疾病 demyelinating disease 神经纤维的髓鞘脱失而神经细胞相对甚少受累的一类疾病。病因不同，临床表现各异，但具有共同的病理特征。这类疾病分为两组，即髓鞘形成障碍型和髓鞘破坏型。髓鞘形成障碍型脱髓鞘疾病是遗传代谢缺陷引起的髓鞘形成障碍，主要包括髓鞘磷脂代谢异常引起的脑白质营养不良，如异染性白质脑病、脑白质海绵样变性、肾上腺白质营养不良等。髓鞘破坏型脱髓鞘疾病是后天获得的脱髓鞘疾病。病因为：①免疫介导，如多发性硬化、急性感染性多神经根神经炎。②病毒感染，如进行性多灶性白质脑炎、亚急性硬化性全脑炎。③营养障碍，如脑桥中央型髓鞘崩解症。④缺氧，如迟发性缺氧后脱髓鞘脑病、进行性皮质下缺血性脑病。一般临床上诊断脱髓鞘疾病时多指免疫介导的脱髓鞘疾病，包括多发性硬化、急性感染性多神经根神经炎等。 |
