| 气管切开 tracheotomy 在颈部中线切开气管并置入气管套管以保持患者呼吸通畅的一种手术。适用于：①喉部炎症、异物、外伤、肿物等所致喉阻塞，短时间病因不能除去者可施行气管切开，解除吸入性呼吸困难。②脑血管意外、肺心病等全身性疾病所致昏迷、咳嗽反射消失，为清除下呼吸道积存分泌物以保持呼吸道通畅。③施行口腔、咽喉等大手术，为保证术中及术后呼吸道通畅。④肺心病、肺功能减退时伴下呼吸道感染，为减少死腔，增加有效换气量时可施行气管切开。进行气管切开术，紧急时可先经口插入气管镜及麻醉插管，或先行环甲膜切开，再行常规气管切开。手术时，患者取仰卧垫肩头后仰位，于颈前下部正中切口，中线分离肌肉、甲状腺峡部，于气管2～4环切开1～2个气管环，置入气管套管。气管切开可有出血、感染、纵隔气肿、气胸等合并症。气管切开后，对病人应有专人护理以防脱管和气管套管阻塞危及病人生命。 |
