冻融胚胎移植


  在控制促超排卵药物刺激下,一次采卵可获得多个卵子,体外受精成功后可获得多个胚胎,而胚胎移植一次不宜超过4个,以避免多胎妊娠。其他多余胚胎可冷冻储存;如本刺激周期未获妊娠,可在以后自然周期适时解冻后移植,从而增加了妊娠的机会。另外,为了避免卵巢过度刺激综合征,本刺激周期不移植,冷冻胚胎储存备自然周期适时再移植。目前大都采用胚胎在4~8分裂球阶段冷冻,慢速冷冻法,以丙二醇为细胞内冷冻保护剂,蔗糖为细胞外保护剂,在液氮罐-196℃内储存。解冻采用快速法,冷冻保护剂在室温下对胚胎有毒性,应逐渐脱掉。将冻融的胚胎继续培养2小时后,如50%分裂球仍存活,可行移植。有少数专家采取胚泡期冷冻,因其含多个细胞可以耐受冻融过程的损伤;但在体外培养的条件下,很少胚胎可达到胚泡期。冷冻保护剂为甘油及蔗糖。近来还有作者试用了超速冷冻法,用二甲亚砜为冷冻保护剂,从室温下直接投入液氮罐,亦有初步成功的报道。
