osteochondritis dissecans
1884年Knig首先描述此病并使用此名,沿用至今。系关节凸面软骨下骨质和软骨小块局限性缺血坏死。病因不明,与遗传、内分泌紊乱、感染、创伤、局部血循环障碍等因素有关。青少年和年轻成人多见,男性较多,常为单侧,多发生于股骨内髁、股骨头、距骨滑车、肱骨小头、跖骨头等。早期仅有关节间歇性酸痛和积液,X线摄片见到局限性关节软骨下高密度骨块,与周围骨质有一透明线分隔。若坏死骨软骨块脱落进入关节腔成为游离体,则关节出现交锁症状伴有活动障碍,X线摄片可见关节软骨下凹碟状骨缺损,关节腔内有游离骨片。早期可固定关节,避免负重。骨片较大者可局部钻孔及内固定。若已形成游离体并经常出现交锁症状,可手术摘除之。
