| 产科药物应用原则 principles of drug use in obstetrics 孕妇在妊娠期用药时，大多数药物可通过胎盘从母血进入胎儿，因而孕期用药必须遵循以下原则：①用药必须有明确指征，对疾病治疗有益，不要滥用和自选药物。②特殊药物虽然对胎儿可能有影响，但可治疗某些危及母儿健康和生命的疾病仍须给药。③可用可不用的药物尽量少用或改变其用药途径，如滴虫性阴道炎应用灭滴灵治疗，在孕12周前不治疗，12周后以局部用药为宜。④用药应在医生指导下，严格控制剂量，合理用药，及时停药。 妊娠期用药的注意事项：①抗生素类。青霉素、头孢菌素族药物对孕妇、胎儿安全。四环素属明确致畸原，孕期禁用。链霉素影响胎儿听神经，孕期最好不用。氯霉素引起灰婴综合征，故不用。②激素类。雄性激素可使女性胎儿男性化，人工合成孕酮结构与睾丸酮相似，故保胎时要用天然孕酮。长期服用口服避孕药过程中妊娠则需中止。③镇静药。胎儿出生前若给孕妇服用吗啡类和大量安定，生后肌张力降低。④抗惊厥剂。苯妥英钠为弱致畸原，有报告用药者较未用药的畸胎率高2～3倍。⑤酒精。易于蓄积，产生胎儿酒精中毒综合征（小头、特殊面容、智力低下等），故妊娠期不用酒精治疗，更不能酗酒。 |
