肺实质内气体被液体取代。X线表现为片状或段、叶分布的密度增高影，肺体积变化不著。肺实变原因很多，造成肺实变的液体来源主要有三种：①气道黏膜上皮分泌过多，主要见于细菌、病毒和霉菌引起的感染；②毛细血管浆液漏出增多，见于左心力衰竭时肺静脉压力升高所致的肺水肿状态，水肿从肺门开始逐渐向肺外围扩展，呈蝴蝶状分布；③吸入液体，可由于溺水或呕吐误吸入肺内所致。
